Please fill this contract out in it’s entirety. Contracts that are incomplete will not be accepted, and we will ask our clients to sign and resubmit a completed contract. Your contract can be filled out with a stylus or finger on some devices, or with a mouse on some devices.

    I, , and B's Pet Delivery agree to the following:

    1. All dogs must be at least 8 weeks of age to be transported.

    2. A valid health certificate issued by a doctor of veterinary medicine dated within 3 days of transport must be obtained and travel with the pet. We recommend the 10 day interstate health certificate. WE CANNOT TRANSPORT YOUR PET WITHOUT THIS CERTIFICATE.

    3. The health certificate must be available at pick up; we cannot wait for the owner to obtain it when we arrive. We keep original for our records as mandated by USDA.

    4. A current rabies vaccination certificate must travel with your pet (this applies to pets over 16 weeks of age).

    5. We need a copy of shot records. Please do not send originals. We keep this copy for our records. Puppies require a clean (negative) fecal exam.

    6. Please send a supply of food for your pet(s), appropriate for days traveled and preferably bagged in Ziploc’s for each day/feeding plus some extra. We will supply bottled water, bowls, and we can supply bedding (for an extra charge). Our space is limited so we ask that no additional food or supplies be sent with driver.  Also, due to limited space, crates, totes, and other large objects are usually not able to be stored and transported (see driver for specific restrictions). Please label your items with pet’s name.

    7. Should your pet require medical attention during transport, we are authorized to provide such care as is deemed necessary. Should emergency care be necessary, we will make every effort to contact you prior to taking any emergency action. However, if you cannot be reached we will use our discretion to take such steps necessary to insure the well-being of your pet(s). All charges incurred for veterinary care will be your responsibility.

    8. For the safety of our clients and their pets, we will not transport a vicious animal. All pets must be socialized and friendly. All pets must be lead (leash) trained.

    9. Any information given over the phone is an approximate by the schedule at that time. IT IS NEVER A GUARANTEE. Please go by the 3-7 day window you are given.

    10. Any coast to coast transports are estimated at a 5-7 day transport time from the date of pick up.  All other transports will need to speak with the driver for an estimate of delivery (This is dependent on the schedule that we are on and can vary). Rescheduling: In the event that you find yourself needing to reschedule your transport within 72 hours of the scheduled pick up, you will be charged a rescheduling fee of 50% of the total agreed cost. It is important that you notify us immediately upon your change of plans.

    11. The driver will inform you if there are any delays in route, due to weather or other reasons beyond our control.

    12. If there is a delay with the Customer at pick up or the Delivery Point, we will charge a Hourly rate of $40 for wait time.

    CANCELLATIONS:

    Cancellations on Shared Rides
    10 days or more, prior to the scheduled pick up date will be refunded minus a $100 cancellation fee .
    Cancellations made after payment has been processed will receive a refund of the cost of the transport less a $100 cancellation fee.
    If you cancel within the 7 days prior to your pick up date, you will be charged the full transport fee and no refund will be given.
    In the event that a Deposit is made ( company discretion), a 50% NON- Refundable down payment is required.

    Private transports cancelation Policy
    30 Days prior to the scheduled pick up the customer will be charged a 20% cancelation fee
    15 Days prior to the scheduled pick up the customer will be charged a 50% cancelation fee
    7 Days prior to the scheduled pick up the customer will be charged a 100% cancelation fee and no Refunds will be given

    Declaration of Valuation

    Every pet boarding our vehicle must have a Declaration of Valuation signed and agreed to before boarding. We do understand that every pet is priceless to you, however failure to state a value will result in an automatic value of $0 in the event of an incident.
    B's Pet Transport covers dogs and cats up to the first $500 declared value at no additional cost to the client.

    We do not cover small birds, reptiles, rabbits, turtles, tortoises, mice, rats, ferrets and other alternative pets. These pets are automatically valued at $ 0

    Additional Valuation Charges that will be added to your invoices.

    All valuations over the first $500 will be charged at the following rate:

    $10.- per $100 per pet with a max value of $2000 per dog & large/exotic birds, $1000 per cat.

    $15.- per $100 for value between $2000 & $4000 for registered pure breeds only as show pets or breeding stock.

    Valuation does not cover in the event of old age, known or unknown illness or undisclosed habits
    (ie, chewing/eating, biting, escaping) that results in or causes death.

    PICK UP:

    1. The driver will contact you 24-48 hrs prior to arrival.

    2. If no person is present at time of transport and/or the above described paperwork is not in order at the time of pick up or delivery, we may have to continue without your pet.  You will be charged for the full amount of the transport cost. We recommend that you make alternative arrangements in case the responsible party is unavailable.






    DELIVERY:

    1. The driver will contact you 24-48 hrs prior to arrival.

    2. The party who ordered the transport will be responsible for any extra charges. It is highly recommended that alternate arrangements be made in case the person responsible for receiving the animal is unable to do so.

    3. The receiver must be available 24 hours a day. We will make every effort to arrive during the hours between 6 AM and 10 PM. If no one will be available for drop off, it is the client's responsibility to make other arrangements such as a kennel/boarding facility.

    4. Kennel/boarding facility costs are responsibility of the client. If client needs assistance finding a kennel, they can contact B's Pet Delivery.

    5. Each client is given a scheduled one week window of time for pick up. We may need to make a pickup or delivery outside of normal business hours to keep on schedule.

    6. Due to conditions beyond our control we cannot guarantee an exact date and/or time for pick up or delivery.






    RIGHT OF REFUSAL:

    1. Our drivers are given the right to refuse acceptance of your pet(s) because of failure to follow the above mentioned terms, and/or your pet(s) is found to be vicious/threatening or sickly.

    LEGAL:

    1. If any Legal Issues/Disputes arise, I reserve the right to resolve them if necessary in Yellowstone County, Montana, as the venue for resolving any Disputes that Montana law controls.

    EMERGENCY MEDICAL CARE CONSENT:

    1. Should emergency medical care be necessary, client consents to treatment by the nearest veterinary clinic available and assume all charges.

    By signing this contract and/or making payment you are agreeing to all the above.
